Welcome to our website www.lookouthealth.com (the “Website”) and the Compass platform  compass.lookouthealth.com (the “Platform”). We refer to the Website and Platform together as the “Services.”

Use of Services.  This Privacy Statement is designed to inform you about our practices regarding the use, collection, and disclosure of personal information that you may provide via the Services.  Please note that this Privacy Statement does not apply to any personal information we receive from your employer, sponsor, or representative in connection with your use of the Services. That information is subject to their privacy policies. If you have questions concerning how they collect and use your personal information, please contact them directly.

The Services. The Services are owned by Lookout Health Corporation, a Delaware corporation (“Lookout,” “we,” or “us”). The Services are intended for use by persons who are residents of the United States.  The Services are not intended to be used by persons under the age of 18.  If we learn that we have collected personal information from a child, we will endeavor to delete that information.

About Your Information.  The Services may ask you to submit “health information”. Health information is any information in a medical record or designated record set that can be used to identify an individual, and that was created, used, or disclosed in the course of providing a health care service.  You will be informed what information is required and what information is optional.  We have taken steps to protect the health information you provide via the Services from loss, misuse, and unauthorized access, disclosure, alteration, or destruction.  Lookout Health maintains your privacy rights in accordance with the applicable provisions of the U.S. Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), applicable federal and state laws, and the terms of any specific consent for such services that you acknowledge. 

How We Use Your Information.  We use personal information to operate and administer our Services.  This includes providing and maintaining the content and functionality of the Website and Platform, responding to your inquiries, communicating with you, managing our relationship with you, and providing and administering any testing services we perform for you. We also use personal information to develop and improve the Services, including to analyze how you interact with our Services, to prevent fraud, criminal activity, or misuse of our Services, and ensuring the security of our IT systems, architecture, and networks (including troubleshooting, testing, system maintenance, support, and hosting of data).

  • Health Care Providers. To provide our Services to you, it may be required for us to provide your information to our affiliate third-party health care providers including, but not limited to Cynergy Wellness[1], Lookout Health Medical, P.C., Quest Diagnostics, TruDiagnostics, 20/20 Gene Systems, or others with whom we may contract as service providers.  Any business affiliate with which we share your personal information may only use the information as provided herein, and as provided on the Website or the Platform.  The Website may contain links or references to other websites to which this Privacy Statement and Terms of Use does not apply.  We encourage you to read the Privacy Statement and Terms of Use of every website you visit.
  • Vendors and Service Providers. To assist us in meeting business operations needs and to perform certain services and functions, we may share personal information with vendors and service providers, including providers of hosting services, cloud services, and other information technology services providers, email communication software, customer relationship management and customer support services, and web analytics services. Pursuant to our instructions, these parties will access, process, or store personal information in the course of performing their duties to us.
  • Business Transfers. If we are involved in a merger, acquisition, financing due diligence, reorganization, bankruptcy, receivership, sale of all or a portion of our assets, or transition of service to another provider (collectively, a “Transaction”), your personal information and other information may be shared in the due diligence process with counterparties and others assisting with the Transaction and be transferred to a successor or affiliate as part of that Transaction along with other assets.
